Substitute Gonçalo Ramos scored the decisive goal in stoppage time, heading home a cross from Rafael Leão in the 94th minute to seal the dramatic victory.
Cristiano Ronaldo also made history by scoring his first-ever goal in the knockout stage of a FIFA World Cup, helping Portugal overturn the early deficit.
Portugal will now face European champions Spain, who advanced with a convincing 3-0 victory over Austria to record their first men’s World Cup knockout win in 16 years.
Meanwhile, Switzerland also progressed to the Round of 16 after defeating Algeria 2-0.
